    • Pet's Species/Breed/Weight/Age
    • Description of the presenting issue and when it started
    • Any recent changes in your pet’s lifestyle or behavior
    • Current medications- bring with you
      **If your pet ingested a plant, toxin, household chemical or human medication you are advised to call Pet Poison Control at (855) 764-7661.**

      • Have the plant, toxin, chemical or medication information readily available as well as know the quantity possibly ingested.
      • They will provide guidance before coming to the hospital which can be critical in timing as well as recommendations for you and the veterinarian to start treatment immediately.
